Communicating With a Loved One With Alzheimer's Disease

The Alzheimer’s Association has some effective tips for communicating with a loved one suffering from Alzheimer’s disease or another form of dementia.  As the ability to find the right words or to speak at all is one of the effects of the devastating disease, it is helpful to learn some tips to make the communication process easier.    

Typically, in the early stages of the disease, individuals repeat themselves often and can easily lose their train of thought.  They may start to avoid speaking and have trouble following a conversation, and may even use gestures rather than words.  Experts suggest at this stage that you make sure to address the person directly, don’t speak through a caregiver, and allow them plenty of time to answer you back.  

The middle stages of the disease can signal lots of changes and usually lasts the longest.  If you wish to speak with the individual, make sure to do it in a quiet space without any distractions.  Your questions should be short and only require a simple yes or no answer, and again, look directly at the person.  In addition, you can demonstrate the task or give clear, step-by-step instructions.  Also, use a soft voice and calm demeanor, and above all else, be patient and loving.       

In the late stages of the disease, which can last from a few weeks to several years, the Alzheimer’s Association suggests using nonverbal forms of communication.  When entering the person’s presence, always identify yourself and use the sound of your voice to convey the meaning of your words.  Experts recommend using touch, sight, sound, smell and taste to communicate, and as always, treat the individual with respect and dignity.

As the Temperatures Rise - Remember to Stay Hydrated

With summer bringing hotter, steamier temperatures, it is a good time to remind everyone, and especially seniors, to protect themselves from dehydration, which happens when our bodies lose more fluid than it takes in. Dr. Samuel Keim, a professor and head of the department of emergency medicine at the University of Arizona, explains one of the reasons adults 70 and older are more at risk for dehydration is “a diminished thirst mechanism and a decreased ability to compensate for high-temperature environments.” He adds this means seniors are less likely to feel thirsty or realize they are becoming dehydrated.

Moreover, many seniors suffer from medical conditions such as respiratory ailments, diabetes, high blood pressure and heart disease exacerbating the problem. Also, medications like anticholinergics or antihistamines disrupt the body’s natural reflexes and reduce its heat tolerance causing a person to sweat less. Other problematic drugs include diuretic medications that increase the amount of water and salt expelled from the body as urine or beta blockers that interfere with the body’s natural response to becoming dehydrated.  

Heat-exhaustion symptoms can include fatigue, thirst, profuse sweating, lightheadedness, cramps, nausea and weakness, and need to be addressed immediately as they can lead to heat stroke, which can be deadly. Treatment includes getting the person out of the heat into a cool location and removing heavy clothing. Spraying the person with water and directing a fan on them also helps. Seek emergency attention if the person is confused or fainting but continue to treat the person until help arrives.

Have You Noticed Memory Issues in a Loved One?

It is worrisome to notice changes in a spouse, parent or another loved one’s health. If those changes involve the person’s memory, thinking or behavior, it can be hard to know what to say or do. The Alzheimer’s Association explains while you may be unsure on how to best approach the issue, these changes could be a sign of a significant health concern. They offer the following advice on how to sort through the changes. 

The first thing is to think about what is causing your concern and how out of the norm is it for the person? Then consider what else may be going on in the person’s life such as changes in their physical health or other stress factors that could impact their mental state. Remember depression and diabetes can cause changes in a person’s mental acuity. 

Now take a look at the Alzheimer’s Association’s 10 warning signs of Alzheimer’s and other dementia diseases. Do these common signs seem to be in line with the type of things you are worrying about? If they do, you may want to enlist some help from other trusted family members or friends.

Start by asking them if they too have noticed any changes in the person’s behavior. If there is an agreement on something being amiss, experts say it is best if one or two people are selected for the task of discussing the problem with the person as a crowd may be too threatening.

Good conversation starters can be something like, “I have noticed some changes in you, is everything okay?,” or use a specific example of something troubling you have noticed the person doing. Offer to go to the doctor with the person; and if the first talk is not productive, try again. Keep in mind, the Alzheimer’s Association’s website has a lot of resources and support so take advantage of the help.

Researchers Report a Link Between Gum Disease and Alzheimer's

Researchers at Tuft University reported a link between gum disease, also known as periodontitis and periodontal disease, to an increased risk for Alzheimer’s disease. Periodontitis is a bacterial infection within the soft tissue of the mouth, and “about 47.2% of U.S. adults over 30 and 70% over 65 have gum disease,” according to the Centers for Disease Control and Prevention (CDC). 

The study, which was done with mice, discovered the correlation, but researchers hope the findings can be used to someday help reduce cases of Alzheimer’s disease. The authors cite Fusobacterium nucleatum (F. nucleatum), a bacterium that causes gum disease and is already linked to kidney disease, diabetes and coronary issues to be the problem. 

The lead author, Dr. Jake Jinkun Chen, professor of periodontology and director of the Division of Oral Biology at Tufts University School of Dental Medicine explains the purpose of the study is to identify and characterize the association of induced periodontal disease with Alzheimer’s disease. He says as F. nucleatum generates “inflammation both locally and systemically, it affects the Alzheimer’s disease phenotype, as Alzheimer’s itself is an inflammatory disease.” Additionally, since the bacteria migrate and penetrate the brain, it can worsen the symptoms and signs of Alzheimer’s disease. 

Researchers plan further studies in humans and say although there is not a proven link yet; good oral health benefits our overall health. They also say to decrease our risk for Alzheimer’s, it is important to maintain a healthy diet and exercise.

Use Caution About What Websites You Trust for Medical Advice

Let’s be honest, many of us rely on the Internet for medical advice. While it can supply good answers, doctors also caution you have to be careful what sites you trust. There are many websites providing good medical information but there are also sites with misleading or even dangerous advice.

Dr. Michael Langan, an internal medicine physician at Ohio State University Wexner Medical Center, cautions anyone with a blog can dispense information and advice; and a good way to avoid misinformation is to ask your doctor to recommend the sites they trust. Additionally, the National Institute on Aging recommends using health websites sponsored by Federal Government agencies. They say a good place for reliable, up-to-date information is the National Institutes of Health, but at the same time, remind everyone that the Internet is no substitute for seeing your doctor or other health professional who can give you advice that caters to your specific situation. 

Other things to look for include name recognition such as the Mayo Clinic or the Cleveland Clinic. Moreover, remember sites that end in .gov, identity U.S. government agencies; .edu, educational institutions; and .org, usually designates nonprofit organizations (such as professional groups; scientific, medical, or research societies; and advocacy groups). A .com is a commercial site.

The NIH says to look for the date of the information (often can be found at the bottom of the page) and see if the authors and contributors are identified. If the author is listed, what is his/her background and what is their goal in writing the article or the intent of the organization it is written for. Is there any financial motive for the information? Check out the About Us or Contact Us pages as they provide background information about the organization.

Are You Having Memory Problems?

Do you feel as if memory problems are interfering with your life or have family members started to notice? Medical experts say there may be a distinction between a person noticing memory loss and others becoming aware of it as this could signal a bigger problem and be an early sign of dementia.  

The Harvard Medical School explains the best place to start understanding the problem is to talk to your doctor. While they may not have the proper training to do extensive screening for MCI (mild cognitive impairment), they can rule out other things causing your memory loss such as medications you take or a recent head injury. Steps you can take on your own to improve cognitive function include getting enough sleep each night, eating a healthy diet, exercising regularly, and reducing stress.

After your own physician has completed an examination, and nothing appears to explain your memory loss, it is time to see a specialist. They, or your local Alzheimer’s Association chapter or hospital should be able to recommend a behavioral neurologist or a geriatric psychiatrist. He or she will probably start with a series of written, on the computer, or oral tests to “assess memory, reasoning, attention, language, visual functions, motor functions, and social functions (such as empathy and knowing how to behave appropriately in social situations).”

They will check for “abnormalities in muscle strength, coordination, reflexes, senses, eye movement, and the pupils’ reaction to light, which can signal abnormalities in specific areas of the brain.” In addition, the doctor will check for Parkinson’s, a stroke or tumors, and will probably schedule a hearing and vision test. If indicated, you may have advanced testing such as PET scans or your cerebrospinal fluid (CSF) biomarkers checked for signs of Alzheimer’s disease.

As You Age, Protect Your Eyes

Did you know it is estimated 895 million people worldwide will have some level of vision impairment by 2050. The Lancet reports it signals a 150 percent increase over the next 30 years and includes loss of vision from cataracts and glaucoma. Experts say while many vision problems can be treated, in some cases they can also be prevented by adopting simple, healthy habits. 

If you have already gotten into the habit of wearing sunscreen and a sun-blocking hat when you are outdoors for any period of time, optometrists say you need to complete your protection by making sure to wear protective sunglasses. Exposure to UV increases our risk for cataracts and your sunglasses should have a label reading “100 percent protection from UV and UVB rays” or “100 percent protection against UV 400.”  If you are outside working for long hours, consider wrap-around style sunglasses, and if you are doing work such as trimming bushes or chopping wood, be sure to wear safety sunglasses.   

As the risk for eye diseases increase as we age, the National Institute on Aging recommends having your eyes checked regularly by an ophthalmologist or optometrist. For those over age 50, you should get a dilated eye exam every year or as recommended by your eye care professional. Everyone over the age of 60 should get a dilated eye exam every year or two. Regardless of age, anyone with diabetes or high blood pressure needs to get a dilated exam at least once a year.

Other ways to protect your eyes as you age is to eat a balanced diet with lots of fresh fruit and vegetables. Additionally, stop smoking, maintain a healthy weight and stay physically active. If you do have high blood pressure or diabetes, you need to learn how to manage them and follow your doctor’s guidelines.

Most Skin Cancers are Curable if Treated Early

The Skin Care Foundation reports the most common cancer in the US is of the skin and strikes one in five people by age 70. The good news is 99 percent of all cases are curable if they are diagnosed and treated early enough. However, to spot skin cancer, we need to look for it, which is why doctors recommend adults to do a routine skin check once a month and have a skin check with a dermatologist at least once a year.  

Since we are the best ones to notice any changes in our skin, it helps to know what to look for. Experts explain skin cancers appear in many shapes and sizes, and warning signs are associated with the different types of skin cancers. Basal cell carcinoma (BCC) (also called basal cell skin cancer) is the most common type of skin cancer accounting for 8 out of 10 cases. Basal cell cancer starts in the basal cell layer (the lower part of the epidermis).

The Mayo Clinic explains that squamous cell carcinoma (SCC) develops in the squamous cells that make up the middle and outer layers of the skin. While not life-threatening, it can be aggressive and typically occurs on sun-exposed skin including the scalp, the backs the hands, ears or lips, but can also occur on any part of the body. 

Melanoma is the most serious type of skin cancer and generally develops anywhere in the body that has been exposed to UV light. Additionally, it may be found in spots not typically exposed to sunlight such as the bottom of the feet. The Mayo Clinic says melanoma can start as a change in an existing mole or a development of a new pigmented or unusual-looking growth on the skin. 

It is important to avoid UV light from the sun or tanning beds as it is one of the top culprits to damaging and aging the skin as well as causing skin cancer. Make an appointment with your healthcare provider if you have a sore or scab that doesn’t heal in about two months or a flat patch of scaly skin that doesn’t go away.

As We Age, Our Thinking Process Begins to Slow Down

Many of us find ourselves wondering if our brain is working as efficiently as it did when we were younger. Research shows most likely it is not. Various studies suggest memory problems often “reflect a slower processing speed and poor encoding and retrieval of new memories as a result of diminished attention.” However, while normal aging causes seniors to process new information slower; judgement and reasoning powers remain largely intact. 

Researchers say in the same way we cannot jump as high or run as fast as we did when we were children; our thinking process also begins to slow down. By the time we are in our 50s or 60s, this can interfere with remembering things, solving problems or learning new information. Structural changes in the brain regions involved in memory processing such as the hippocampus and the frontal lobes all experience anatomical and neurochemical changes as we age. Concentration can be affected by the natural loss of receptors and neurons. Outside factors such as not getting enough sleep or a hearing loss can exacerbate the problem, and daily aches and pains can impact our ability to concentrate as do the medications we take to ease the pain.

If an older person does not make a point to really concentrate when taking in new information, it can cause problems with recall as they never really learned it in the first place. Experts offer practical, commonsense tips to help you more reliably process information and improve your ability to focus. Examples include playing brain games, physical exercise (it helps send more blood flow to the brain), getting out in nature, listening to music, sleeping at least seven to eight hours a night or using relaxation techniques such as meditation or mindfulness.

Falls in Older Adults

When an older adult trips and falls, it can be life changing. Consequences include serious head injuries, hip fractures and broken bones, and in the worst cases, fatalities. The Centers for Disease Control and Prevention (CDC) reports 3 million seniors are treated in emergency departments for fall injuries each year. Furthermore, more problems can arise as many seniors need several months to recover, which can cause a decline in strength and mobility as well as mental agility.  It can also lead to the loss of independence and isolation. 

Statistics show most falls happen for a handful of reasons and being forewarned may help prevent one. The environment, especially the person’s home and surrounding outdoor area is one of the biggest culprits. Danger spots include slippery bathtubs, high mattresses, steep stairs, poorly lit hallways, loose rugs, obstacles in walkways and clutter. Experts say a little bit of time spent addressing these problems with things like non-slip bath mats and grab bars, more lighting, removing clutter, etc. can help prevent a fall. 

Another risk factor for a fall is chronic health conditions that affect balance and mobility. Arthritis, heart disease, diabetes and high blood pressure can mean a person might experience dizziness or be unable to control muscles to stop a fall. 

Growing older also puts us at risk for diseases like glaucoma, age-related macular degeneration, diabetic retinopathy and cataracts, which in turn, leads to more accidents. Solutions include having yearly eye exams and making sure your eyeglass prescription is up to date.

Experts say it is important for seniors to stay as fit as possible as aging affects muscle strength, flexibility and coordination. Check with your doctor for information on balance and strength exercises. If a medication is making you dizzy or drowsy, talk to your doctor about it. There may be another drug just as effective and doesn’t have the same side effects.
