Enrich the Life of a Loved One With Alzheimer’s Disease

An article in the online publication, Medical News Today, suggests activities that can provide a person with Alzheimer’s disease a sense of normality and to help enrich their life. A study from 2020 reported meaningful activities are important for those with dementia as they can improve both psychological and physical health.

One suggestion of an indoor activity is to sit with the person and look through photo albums of family together. This is an effective way to engage the person and is typically enjoyable. Another relaxing activity can be listening to the person’s favorite music; or if they are more hands-on, do an arts and crafts project. Playing an instrument has much the same calming effect. 

Researchers say studies show knitting and crocheting to be calming activities for those with Alzheimer’s. Not only does it help reduce anxiety and depression, but the person gets a sense of accomplishment. Moreover, done within a group setting, it can lessen feelings of loneliness and isolation. Routine things like daily chores can also be comforting and help the person feel like they are a contributing member of the family/group.     

Experts say outdoor activities (weather permitting) are important to the person’s overall health and well-being. One area of research for those having dementia includes therapeutic gardening. Data shows gardening can improve a person’s emotional and cognitive state, increase engagement and reduce anxiety. 

Other healthy outdoor activities include walking in the park (getting back to nature is good for all of us) and feeding the birds. If the person uses a mobility aid such as a walker or cane, be sure to have them use it. The same goes for wearing good-fitting walking shoes. 

The Alzheimer’s Association suggests keeping to a routine each day is helpful; as well as offering other promising ideas worth considering. In addition, they say to remember everyone is different, and if an activity does not seem to be to their liking, try something else. Most importantly activities should be those the person enjoys.

A Living Will Ensures Your Care Choices Are Followed

Many times people put off creating a living will, also known as a health care power of attorney, because they don’t understand how important it is to have one; or they fear they are giving up too much control over their life. The medical community explains the reason you need one is to ensure you get the type of care you want when you can no longer speak for yourself.

The Harvard Medical School says before you create a living will there are a few things to consider including separating fact from fiction. To start with, think about what quality of life means to you. Sometimes having extraordinary measures taken to save your life doesn’t factor in where such treatments will leave you. Having a living will can help guide your medical team on just such decisions.

Some people worry if they choose to refuse certain life-sustaining medical treatments to prolong life even when death is inevitable that it will invalidate their life insurance policy. Medical professionals explain there is no reason to worry as the recorded cause of death will be from the person’s underlying medication condition. Another common concern is if medical treatment is started, it can’t be stopped. This too is not true as treatment plans can always be updated and revised to address current circumstances.   

Medical professionals want patients to know treatment can be started or stopped. Even if you refuse certain life sustaining treatments, it does not mean you won’t get other types of treatment such as intensive comfort care to help manage pain and symptoms. 

Seniors especially should be aware a new directive has pretty much replaced a DNR (do not resuscitate), which only gives EMS permission to not perform cardiopulmonary resuscitation (CPR). A POLST (physician orders for life-sustaining treatment) can include a DNR as well as provide more instruction regarding medical interventions such as intubation, antibiotic use and feeding tubes. Alzheimer's Disease Can Cause Visual and Spatial Issues

Most of us are familiar with the forgetfulness and confusion Alzheimer’s disease causes. Especially in the early stages, family and friends start noticing the person is constantly repeating themselves and/or forgetting where they put things. However, experts say there are other commonly occurring signs of a serious problem.  

One of those early warning signs can be having trouble navigating and even getting lost in familiar places. A decline in spatial awareness as well as vision problems can cause a person in the early stages of Alzheimer’s to become confused in new places and have trouble identifying objects and people.

Spatial and visual issues may include looking at an object and not seeing it clearly for what it is. Examples could be a person sees a blue floor as water or a dark spot in a rug as a hole in the floor. They may be unable to tell if their house is to the left of them or to judge distances accurately. Some people may mistake their child for their spouse or another relative. 

These misconceptions and misidentifications are caused by damage to the neurons, which are “cells in your brain that communicate with each other and with the rest of your body. Alzheimer’s causes damage to neurons.”  Researchers say they are not sure if the problems are due to impairment to the spatial reasoning part of the brain or damage to the temporal and parietal lobes.

There are things caregivers can do to help a person navigate their surroundings more easily such as removing clutter from the person’s living space and labeling items. Keeping things in the same place is another helpful tip as habit is a strong instinct. If you are out with the person and they look confused or frightened, stop what you are doing, and calmly explain where you are as well as listen to their perspective of what they are feeling.

Reduce Your Risk of a Fall

The Center for Disease Control & Prevention (CDC) reports millions of older adults fall each year—resulting in more than 38,000 deaths in 2021. Of those falls, 3 million seniors are treated in emergency departments for fall injuries; and one out of every five falls causes an injury, such as broken bones or a head injury. With these alarming statistics, experts say it is important for seniors to know their risk for a fall including whether they are taking medications associated with an increased risk. 

The first step is to talk to your doctor to see if you are taking one of these medications, as medications are a common reason for falls. Furthermore, experts say adjusting or making a change in medication is the easiest thing to do to help prevent a fall. If it is determined you are taking a medication known to cause light-headedness, dizziness, sudden drop in blood pressure, or chronically low blood pressure or hypoglycemia (low blood sugar), you and your healthcare team can regularly re-evaluate your need and dosage. 

Another way to mitigate your risk for a fall is to do exercises designed to improve leg strength and balance. In addition, be sure to get an annual eye exam and update your glasses as needed. Lastly check your home for trip hazards (keep clutter out of hallways and stairways) and install grab bars and better lighting where needed.

Do You Bruise Easily?

As we age, many seniors discover new bruises on their body but have no memory of how they got there. The Medical News Today (MNT) explains this may be the result of the skin thinning and blood vessels weakening and/or that some people are just more prone to bruising. It can also run in families. Being a frequent, extreme bruiser includes having many bruises without remembering their cause; regularly developing bruises that take many weeks to heal and bleeding for longer than 10 minutes following an injury.

Excessive bruising and bleeding can be from medications, many of which seniors routinely take. These include drugs like warfarin, heparin, rivaroxaban, dabigatran, apixaban and aspirin. Doctors say even certain herbal remedies such as ginkgo biloba, ginseng, feverfew, large quantities of garlic, ginger, and willow bark can increase the risk of bleeding or cause a change to blood vessels. The same is true for drugs like corticosteroids and glucocorticoids of which one example is prednisone.  

MNT reports liver disease is also associated with excessive bruising. “As the disease progresses, the liver may stop producing the proteins that help the blood clot. As a result, a person may experience excessive bleeding and easy bruising.” Other blood disorders and diseases such as hemophilia, senile purpura, or even vitamin deficiencies can include bruising as a symptom. Experts say consult your healthcare provider if you suddenly begin bruising more easily, have symptoms such as yellow skin, fever and low energy, taking medication and begin bruising, or getting painful bruises that are slow to heal.

Is Palliative Care the Same as Hospice?

To help those dealing with end-of-life issues, the National Institute on Aging (NIA) explains some of the myths associated with palliative and hospice care including sorting out the difference between the two terms. They explain while they are similar, they do differ in a couple of significant ways.

Palliative care is defined as providing specialized care for those living in assisted living with a serious illness. Myths linked to palliative care include the person no longer being able to receive care for their disease. The fact is care can be provided for the illness along with curative care. Another myth is the person will no longer be able to see their primary doctor but will only be treated by specialists. NIA says in many cases, the person’s primary provider will work hand-in-hand with specialists.

The intention of palliative care is to improve quality of life as well as provide care to improve help with symptoms. It also helps the patient understand their choices for medical treatment. A palliative care team may be made up of specialized nurses and doctors along with social workers, nutritionists and chaplains. If a time comes when the person’s care team decides treatment is no longer working, palliative care could transition to hospice care if the doctor believes the person is likely to die within six months.  Another option is the palliative team would continue to help with the emphasis on providing comfort care. 

The NIA explains hospice care provides comfort and quality of life for a person with a serious illness who is facing end of life. The myth surrounding hospice care is the person cannot receive any medications. The truth is a person will receive medications to help manage symptoms but not to treat the disease. Many people think hospice care can only be done in a hospital or nursing facility but experts say it can also be done at home.

Identity Fraud Cost Americans $43 Billion in 2023

Identity theft is the illegal use of a person’s Social Security number, credit card or other personal information to commit a crime. According to a report from Javelin Strategy & Research, identity fraud cost Americans $43 billion in 2023, and this is likely only a fraction of the true cost as these crimes are often not reported. Given these shocking numbers, financial service providers and consumers should be as vigilant as ever about protecting personal identifying information.

Although financial institutions are doing what they can to protect our personal information from those who wish to fraudulently use it, we can also take steps to protect ourselves. The first tip is to never give your bank account and Social Security numbers to people over the phone. Many scammers claim to be from your bank or a government entity and tell the person they are trying to protect them, when in fact they are doing the opposite. Remember the Social Security office or your bank is not going to call you and ask for your number. 

Another way to prevent identity theft is to not reply to phishing emails. These types of emails or text messages can contain official looking logos and promise free services or goods, lottery prizes or government grants. Others may tell you there is a problem with your bank or another company and they need information to settle the account. 

The US Government Office of Justice Programs says in the event of having your identity stolen: 

  • Place a fraud alert on your credit report.

  • Close out accounts that have been tampered with or opened fraudulently.

  • Report the identity theft to the Federal Trade Commission.

  • File a report with your local police department.

They recommend keeping a log of all your correspondence and actions taken to report the incident. This should include dates, names and phone numbers of who you contacted or reported the thief to as well as sending documents via certified mail. Keep a record of any expenses incurred in case you are entitled to restitution when the responsible party is caught.

Older Adults Are at Greater Risk of Dehydration

Dehydration occurs when your body loses more fluid than it takes in. It can happen when we experience an excessive loss of fluids from things like sweating, vomiting and/or diarrhea; or simply not drinking enough. Dehydration can sneak up on people and is a common problem especially in the heat of summer. 

Older adults are even more at risk of dehydration as their thirst sensation decreases with age. This means seniors may not feel thirsty when they really need fluids. Other reasons can include underlying chronic illnesses of the heart, lung or kidneys as well as certain medication such as diuretics.

Maintaining enough fluid in our bodies is essential to good health. Dr. Marc Taub, an emergency physician and medical director of emergency services at MemorialCare Saddleback Medical Center in Laguna Hills, California, states, “Water helps move nutrients and oxygen throughout our system, ensuring cells get the elements they need to function.” 

Therefore it is important to recognize the early signs of dehydration and to address it.  The US News & World Report states these include: 

  • Feeling thirsty.

  • Dry mouth.

  • Lightheaded.

  • Fatigue.

  • Darker-colored urine.

  • Odorous urine. 

Mild cases of dehydration are easy to fix. Just drink a couple of glasses of water or try an electrolyte-enriched beverage like sports drinks or coconut water. If you are outside, get out of the sun and seek a cool location. 

Doctors say since older adults are more at risk, they should take extra precautions to make sure they don’t get into a difficult situation, particularly those with chronic medical conditions or on multiple medications. This would include staying indoors on hot days and drinking plenty of fluids throughout the day. If you feel you are drinking enough fluids but still feel thirsty or experiencing symptoms associated with dehydration, let your healthcare provider know.
