Is There a Difference Between Alzheimer’s, Senility and Dementia?

While most of us have heard of Alzheimer’s, senility and dementia, we may still be confused wondering if they are all the same thing or different diseases. Doctors explain the terms are often used interchangeably but do have different meanings. Dementia describes a syndrome with a wide range of symptoms that impact a person’s ability to perform everyday activities independently. While not an actual disease, the symptoms include memory loss, and difficulty with thinking clearly, making decisions and in some cases, controlling emotions. Dementia can be caused by a variety of diseases or conditions.

Alzheimer’s is a neurodegenerative disease and one form of dementia accounting for 60 to 80 percent of cases. The disease was formally recognized in 1906 and named after the German physician, Aloïs Alzheimer, who first described it. Recent advances in medicine have found the brain can show signs of the disease years before the person experiences any symptoms. 

Dementia is typically irreversible but there are some conditions that can cause symptoms of the disease. These include vitamin deficiency, underactive thyroid gland, depression or stress; and when treated, can reverse the condition. Lewy Body Dementia and Parkinson’s disease, a neurodegenerative disease, may also cause symptoms of dementia as can AIDS, Lyme disease and stroke. 

Senility is another term used to describe the type of memory deterioration associated with the elderly. Experts explain there are two major types of senile dementia: “those due to generalized “atrophy” (Alzheimer’s-type dementia) and those due to vascular problems (mainly, strokes).” Symptoms of senility include a decline in cognitive abilities such as problems recalling information and thinking clearly; and Alzheimer’s is the most common form.

Healthy Snacking for a Balanced Diet

The goal of a snack should be to satisfy your hunger as well as be a balanced addition to your daily nutritional intake. Dietician Lisa Jones says satisfying snacks can be crunchy, creamy, savory or salty and good choices include things like eggs, tofu, whole grains, and fruits and vegetables. 

As a guideline, an adult’s snack should contain approximately 150 to 250 calories. Experts advise diabetics to stick to protein-rich choices and to watch the amount of carbohydrates a food contains as carbs can elevate blood sugar. If you are eating pre-packaged food, check to see what constitutes a serving and the number of calories.

Remember healthy snacks can be tasty such as avocado on whole grain toast. For a fiber-rich snack, how about making chia pudding? The recipe is simple and all you need to do is add 3 tablespoons of chia seeds to a 6 oz. glass of seed-based milk. Add ½ cup fruit and refrigerate for a couple of hours. To get 80% of the daily folate and 50% of the manganese requirement, how about eating a cup of shelled edamame?

Other quick snacks include a handful of nuts and a piece of fruit, or a bowl of popcorn topped with a small amount of olive oil rather than butter. Cut-up veggies eaten plain or dipped in peanut butter or humus is another satisfying, healthy snack. Greek yogurt is loaded with calcium, probiotics and protein and can be added to fruit, a handful of nuts or even to tuna.

Celebrate the Special Moms in Your Life

What Are Advance Care Directives and Why Are They Important?

An advance directive is a legal document giving you a voice involving medical decisions in the event you are incapacitated. Many people shy away from creating such documents as they fail to realize how important they are, and mistakenly worry they are signing away their lives, and/or don’t like thinking about being so ill they are unable to make decisions.    

Advance care planning documents may include a living will, power of attorney and health care proxy. By planning ahead, they guide decisions made on your behalf if you are unable, and suffer from an incurable, irreversible illness, condition or disease determined to be terminal. Living wills specify the kind of medical treatment desired and many simply express the desire to not be given life-sustaining measures that only serve to prolong a person’s death. Specific details may regard care including the use of pain medications, antibiotics, hydration, feeding and the use of cardiopulmonary resuscitation or ventilators. 

A health care proxy is someone designated to express your wishes for medical care and has the same rights you would in requesting or refusing treatment. A durable power of attorney document names a person to act on your behalf if you are incapacitated and gives them the power to make financial decisions. 

The Harvard Medical School reports on some of the myths people have regarding advance care directives. They say you need to realize more care is not always better but can prolong the dying process without respect for quality of life or comfort. Refusing life support does not mean you are committing suicide but rather allowing your medical problem to be the cause of your death. Moreover, refusing life-extending treatment does not mean you are refusing comfort measures for pain management and symptoms. The Benefits of a Colorful Diet

Research shows flavonoids, the chemicals that provide plant foods their bright colors, may help in the fight against forgetfulness and mild confusion many seniors experience. The evidence comes from two long-term continuing health studies from the late 1970s and early 1980s. Participants were periodically surveyed regarding their diet and state of their health including memory problems over a 20 year period. Data was extracted from over 100,000 men and women with an average age of 73.  

The study’s lead author, Dr. Deborah Blacker, a professor of epidemiology at the Harvard T.H. Chan School of Public Health, said they focused on participants’ consummation of two dozen common types of flavonoids. These included carrots (high in beta carotene), strawberries (containing flavone) and apples (containing anthocyanin). Results suggest those who answered “yes” to having memory problems also reported eating a lower amount of flavonoid-enriched foods.  

Researchers found a significant difference between the bottom one-fifth of participants who reported the lowest intake of flavonoids compared to the upper one-fifth who were 19 percent less likely to report forgetfulness or confusion. Additionally, the study reported “higher intakes of Brussel sprouts, strawberries, winter squash and raw spinach were most highly associated with better scores on the test of subjective cognitive decline.” Dr. Thomas M. Holland, a researcher at the Rush Institute for Healthy Aging who was not involved in the study, said “There’s some really good data here with 20 years of follow-up.”

Are You Concerned About a Loved One’s Forgetfulness?

As we grow older, many of us find ourselves becoming forgetful and worry it could be a sign of Alzheimer’s or another form of dementia. Researchers say it is true that the older we get we are more forgetful as at least half of those over age 65 say that they are more forgetful than they were when they were younger, experiencing ‘senior moments’ about things like where they put things or recalling somebody’s name.

The question then becomes when what is the difference between “normal” age-related memory loss and Alzheimer’s disease. Studies find the brain shows warning signs of Alzheimer’s approximately fifteen years before someone experiences symptoms of mild cognitive impairment (MCI), and quite a while before the first real signs of dementia start.

Concerning signs of memory loss include not just forgetting an appointment or someone’s name, but repeatedly forgetting it. Another sign that something more serious is going on is if family members start to notice a loved one’s forgetfulness or confusion. Constantly repeating questions, forgetting the word for something or getting lost in a familiar neighborhood are other signs of a bigger problem. Judgement and planning may also be impaired, and tasks the person used to be good at like balancing a checkbook or following a recipe can become frustrating and difficult. 

To know for sure what is happening, you need to make an appointment with their doctor. They can do a complete medical work-up including looking at the medications the person is taking, and checking for signs of depression and other environmental issues, which could account for symptoms. If the person is diagnosed with Alzheimer’s or a dementia-related disease, there are therapies designed to improve quality of life.

Hearing Loss is Common Among Seniors

The National Institute of Health reports approximately one in three people between the ages of 65 and 74 has hearing loss and nearly half of those older than 75 have difficulty hearing. Hearing loss causes someone a range of problems including difficulty understanding and following a doctor’s advice, responding to safety warnings, and hearing doorbells, phone calls, and alarms. Another all-too common problem is it can be hard to follow conversations with family and friends, and many start avoiding situations where they may be frustrated or embarrassed. 

Medical experts advise asking your primary care physician for a referral to an otolaryngologist, an audiologist or a hearing aid specialist. An otolaryngologist is a doctor who specializes in diagnosing and treating diseases of the ear, nose, and throat. They will try to determine the cause of your hearing loss and treatment options. They may refer you to an audiologist who is trained in measuring the degree of loss as well as recommend treatment, which may include fitting you for a hearing aid. There are three different types of hearing aids and it may take some trial and error to find one that works for you (most can be purchased for a trial period).  

In some cases, doctors may recommend Cochlear implants, small electronic devices surgically implanted in the inner ear that help provide a sense of sound to people who are profoundly deaf or hard-of-hearing.  Other treatment options include hearing assistive devices for your phones to amplify sound; closed circuit systems for churches or theaters; and lip or speech reading classes.

The Five Measures of Heart Health

Medical experts say there are five numbers adults should know that can impact their health. Dr. Deepak L. Bhatt, editor-in-chief at the Harvard Heart Letter and part of the interventional cardiovascular programs at Harvard-affiliated Brigham and Women’s Hospital, explains, "For my patients, I typically look at their blood pressure, blood sugar, LDL cholesterol and triglycerides values, and their waist circumference."  

While there are specific recommendations, doctors also say the ideal values are good goals for most people but your doctor may suggest different numbers based on your age and other health conditions. Current recommended blood pressure numbers are less than 120/80 mm Hg. Systolic blood pressure is the first number and diastolic blood pressure the second number, and together they measure how hard your heart is working and the condition of your blood vessels. High blood pressure (HBP) can damage blood vessels and increase a person’s risk for a stroke or heart attack. 

Your LDL cholesterol numbers should be less than 100 mg/dL and your triglycerides less than 150 mg/dL. A cholesterol test is done through a blood draw after a person has fasted and if both your numbers are high, it increases your risk for a heart attack or stroke. Another important number is your blood sugar levels, which is how diabetes is typically diagnosed. Doctors say blood sugar levels should be less than 100 mg/dL.  

Finally, the last number you should pay attention to for better health is your waistline measurement. Experts say the circumference should be either lower than: less than half your height in inches; or for women less than 35 inches, and for men less than 40 inches. The problem with big numbers is what doctors call “abdominal or visceral obesity” typically an indication of fat surrounding the internal organs. Use a tape measure placed just above your navel (belly button) to get your number.
