Enrolling for and/or making changes to a Medicare plan can be confusing. Medical News Today’s website states that learning how the health program works can help avoid errors, including missing enrollment periods, overlooking costs, and choosing unsuitable plans.

To help you avoid costly mistakes that result in you not getting the best coverage, it is important to understand what you are signing up for. One easy way to start is to learn Medicare terms.  

Medical News Today explains the following terms:

Out-of-pocket cost: This is the amount a person must pay for care when Medicare does not pay the total amount or offer coverage. Costs can include deductibles, coinsurance, copayments, and premiums.

Premium: This is the amount of money someone pays each month for Medicare coverage.

Deductible: This is an annual amount a person must spend out of pocket within a certain period before Medicare starts to fund their treatments.

Coinsurance: This is the percentage of treatment costs that a person must self-fund. For Medicare Part B, coinsurance is 20%.

Copayment: This is a fixed dollar amount a person with insurance pays when receiving certain treatments. For Medicare, this usually applies to prescription drugs.

Additionally, it is important to understand Medicare plans. Original Medicare is Part A (hospital insurance) and Part B (medical insurance). Medicare Advantage is known as Part C and is managed by private insurers. Part D offers separate drug coverage. The first topic the survey looked at is whether a senior’s current home is in reality the best place to retire in. Age Wave said results show a move to senior living can in many cases provide more freedom, more convenience, better care and less worry for older adults. 

One sentiment expressed by many is that they appreciate the fact that if their health needs change, or those of their spouse, the community is equipped to deal with the needs without necessitating a move. Also, if they need to see a doctor, they like that they can just schedule a ride to their appointment. 

Another myth is that people fear they will no longer have purpose in their life in a senior community. However, according to the Age Wave survey, people in CCRC reported at this time of life perhaps there is a better purpose than doing household chores. Residents expressed their satisfaction in the freedom to explore new pathways – “from learning how to paint, volunteering in the community, becoming an expert on healthy cooking, authoring a book, reaching new levels of fitness, to developing rewarding new friendships.” 

Sometimes, seeing the costs, people worry it is too expensive. Again Age Wave says several misconceptions can lead seniors to miscalculate the cost of staying in their home to community living. Strategies for Improving Memory

The National Institute on Aging (NIA) reminds older adults that mild forgetfulness is more often a normal part of aging and not a serious memory problem. They explain serious problems generally involving things like the person asking the same question over and over again and getting lost in places they know well. They may also have trouble following recipes or directions and find themselves confused about people, places and time. 

If you are someone who is forgetting things, there are some steps you can take to feel more confident. These involve several simple things you can do, which will benefit your whole being like getting enough sleep (seven to eight hours a night), not drinking a lot of alcohol; and exercising and eating well. Managing high blood pressure is also important.

Other more practical tips to help with forgetfulness are putting your wallet or purse, keys, glasses and phone in the same place each day. Instead of assuming you will remember, write out shopping and need to-do lists and use a calendar or take notes. NIA says to get involved in activities that focus on using the mind and body and stay socially involved. If you feel depressed for weeks at a time, talk to your doctor. 

Strategies to improve forgetfulness include challenging our brains by learning new things as well as using all our senses. For example, prepare a new recipe that requires lots of spices. If you are eating out, try to identify the flavors and seasonings used in the meal. To help remember new information better such as names, places, etc., try repeating it out loud or writing it down as this reinforces the memory. For example, if you want to remember a new neighbor’s name, say it out loud back to them, like “hi Jack.” Then an hour or so later say the new neighbor’s name out loud again.

Many older adults, especially those in assisted living communities, are no longer as steady on their feet as they used to be.  This puts them in danger of a fall, and a walker, cane or even a wheelchair or scooter is probably advisable.  However, before you start using a hand-me-down walker or buy any-old cane; experts warn that one size does not fit all.  In the case of a walker, to be an effective and safe mobility aid, it is important to have an expert help you pick out the best type and size for your needs.  

As AgingCare.com points out, “The wrong device can negatively affect a senior’s stability, cause posture-related problems and even result in pain. Mobility aids come in all shapes and sizes, so selecting the right equipment and achieving a proper fit usually requires a professional’s assistance and expertise.”  To get started determining what your needs are, experts recommend seeing your healthcare provider to discuss any problems you are having with your activities of daily living (ADLs).  If they recognize a need for a mobility aid, they may refer you to an occupational or physical therapist.  

The following are a couple of basic guidelines to know before you get started.  A cane can help provide balance and stability and is ideal for someone who needs to support no more than 25% of their body weight.  It is most useful for those minimally impaired or with weakness in one leg.   Walkers are available in a variety of models but regardless of the style a “correct height of a walker is measured from the floor to the wrist, and the elbows should be flexed in a range of 15 to 20 degrees.”  Experts also say the walker must be large enough to provide front and side support for the user.  

A motorized scooter may be the answer for someone who has lost the ability to walk any distance but still has an adequate amount of strength in their arms and upper body to stay upright as well as drive the scooter.  Again, there are a variety of models including indoor and outdoor ones as well.  Keep in mind though that most insurance companies and Medicare Part B typically only cover scooters for in-house use.

If you or a loved one is experiencing troubling memory problems, it is important to see your healthcare provider. While there is no cure for Alzheimer’s or other dementia-related diseases, there are steps you can take to mitigate or delay the onset of the disease. 

The Alzheimer’s Association explains, “Physicians and other medical professionals are a central part of the care team of a person living with dementia. Along with other members — which may include family, friends or others — they work to provide support throughout the course of the disease.” Therefore, one of the best ways to ensure optimal care is to honestly communicate with your physician.  

To establish a good relationship with the doctor, a person or family member should take some time to prepare for the visit. Helpful tips from the Alzheimer’s Association include:

  • Document any changes in your health, including your mood, memory and behaviors.

  • Make a note of any changes you’ve noticed since your last appointment.

  • Create a list of current prescriptions and over-the-counter medications.

  • Write down any vitamins or supplements you’ve added to your medication regimen.

  • Make a list of questions you want to ask.

Writing down questions you have beforehand makes sure you don’t forget them, and don’t be reluctant to ask questions if you don’t understand something. The Alzheimer’s Association National Early-Stage Advisory Group, made up of men and women in the early stages of Alzheimer’s or other forms of dementia, put together a list of questions to help others make the most of a doctor’s visit. Questions range from asking about the type of tests used to make a diagnosis, what to expect from the disease, and what type of treatments may be used. When seeing a specialist, ask how familiar they are with caring for patients with Alzheimer’s disease.

Is Fatigue Affecting Your Daily Activities?

You may be one of the millions of Americans who complain about feeling a sense of fatigue that makes it difficult to get on with life. The Harvard Medical school explains there is a distinction between just feeling tired and fatigue, which “includes components that are physical (weariness or weakness), mental (lack of concentration and sharpness), and emotional (lack of motivation or boredom).” 

Some of the causes of fatigue may be easier to identify such as lingering symptoms from an illness like COVID or the flu, or side effects from medical treatments like chemotherapy. Even some medications are known to cause tiredness. When fatigue is ongoing and interferes with day-to-day activities, doctors say you should consult with your healthcare provider. 

Your doctor can look for things like anemia as it can be a reason for a decline in energy. It occurs when a person’s blood has too few red blood cells, or cells without enough hemoglobin that is used to transport oxygen throughout the bloodstream. Heart disease is another cause of fatigue as it can make the heart pump less efficiently and result in fluid build-up in the lungs. This can lead to a reduction in the amount of oxygen able to circulate into the lungs and heart. 

Other causes of fatigue include depression and insomnia that often go hand in hand. However, antidepressants, which are often prescribed to treat depression as well as certain blood pressure medications and antihistamines, can increase tiredness. 

Experts suggest consulting a doctor about fatigue related to medical or mental health issues, but there are self-help methods to manage everyday fatigue. The first is to get into the habit of exercising. Next watch what you eat and try to reduce your consumption of fried and over-processed foods. Following a low glycemic diet may help boost energy. Other tips include staying hydrated, reducing alcohol and watching how much coffee you drink each day.

Walking Can Help Improve Balance

Many seniors know a fall could threaten their independence and way of life. Common injuries include to the head, hip fractures, and in the most extreme cases death. Experts explain balance declines as we age as well as can be affected by medications and medical conditions, lack of flexibility and uncorrected vision problems. This is why medical experts encourage seniors to work on maintaining and improving balance. 

The Harvard Medical School says one of the simplest ways to improve balance is with exercises like walking, strength training and workouts targeted to improve stability. However, before beginning any new exercise regime, consult your healthcare provider.  

When beginning a walking plan, whether you are active or sedentary, experts say to remember minutes walked are more important than number of miles. If you are unused to exercise, start slowly. Also, wear good walking shoes, and if you normally use a cane or walker, use it. Experts say it is safer to walk with a buddy and to bring along a cell phone. Walk in a place with an even surface, and especially in the beginning, stay close to home.  Once you start feeling more confident, add more minutes to your routine. 

For seniors who are already walking, try adding a few more minutes to your program or a more challenging route. Experts say the overall goal should be to do at least 150 minutes a week. To add strength training exercises and workouts to improve balance, ask your doctor for advice on how to get started.

Managing Mild Cognitive Impairment With Lifestyle Changes

Mild cognitive impairment (MCI) can be a precursor to dementia but does not always progress to dementia. Medical professionals report 10% to 40% of MCI cases go on to become dementia, and a good percentage of those become Alzheimer’s disease. On the positive side, research shows as many as 30-40% of people with MCI get better. As with Alzheimer’s disease and other types of dementia, there is no cure for MCI but there are some therapies which have been approved to treat early Alzheimer’s to slow disease progression.  Doctors also say there are steps we can take to mitigate or minimize the effects of MCI. 

MCI is just as its name suggests. The impairment is mild and does not affect a person’s ability to care for themselves or take away their independence. Steps a person should take to protect their cognitive health include quitting smoking and moderating or eliminating alcohol consumption. If you do have hypertension (high blood pressure), high cholesterol levels or diabetes, it is important to treat these conditions and manage them through medications, diet and healthy lifestyle habits. 

Other things you can do to thwart, delay or minimize the effects of MCI are to maintain a healthy weight and exercise, which is good for our brains as well as our bodies. Avoid junk food, highly processed foods and foods high in sodium; and reduce the amount of red meat in your diet. Isolation has also been linked to a higher risk for dementia, so it is important to stay social.

Gingivitis: Prevention and Treatment Methods

Gingivitis is a common periodontal disease and can become a serious problem if allowed to go untreated. The disease attacks the tissue at the base of the teeth, called gingival tissue, and causes swelling and inflammation. It occurs when food particles and other substances are left on the gum line where sticky plaque and hard tartar forms at and below the gums.

Dr. Tien Jiang, assistant professor in oral health policy and epidemiology at the Harvard School of Dental Medicine, says with the correct oral care gingivitis can be controlled with proper teeth brushing and routine dental cleanings. If left untreated, inflammation from gingivitis can lead to an irreversible form of gum disease called periodontitis, and damage the tissue and bones supporting the teeth. If your dentist suspects periodontitis, they can use x-rays to see how far the disease has advanced. 

Studies have linked inflammation of the gums to a higher risk of many serious diseases including heart, stroke and dementia. Poor dental hygiene like not flossing, smoking, misaligned teeth, diabetes that is not well-managed, and infections in other parts of the body can put you at higher risk for gum disease.    

While early gum disease usually does not cause symptoms, as the disease progresses; signs include sore, red, sensitive, or swollen gums. Gums that bleed when brushed or flossed, mouth sores and bad breath are other symptoms. 

Dr. Jiang says prevention and treatment begins with home care and should include brushing twice a day and flossing once a day. Pay attention to areas where plaque tends to grow such as between the teeth and at the gum line. If your dentist visits show lots of plaque buildup, they may recommend using anti-plaque or anti-tartar toothpaste or mouth rinse. 

At the dentist’s office, your dentist or dental hygienist is equipped to remove plaque deposits before gingivitis can cause more serious problems. For cases where inflammation is below the gum line, they have special tools to clean the area. Your dentist’s office is also a good place to learn how to properly care for your teeth.

The Impact of Probiotics and Prebiotics on Digestive Health

The last several years have brought probiotics and prebiotics into the spotlight. However, it can be confusing as to what the difference is between the two and the role they play in our bodies. Experts say both are important to our health; probiotics are beneficial bacteria, while prebiotics are food for these bacteria.

In other words, “Probiotics are live bacteria found in certain foods or supplements and can provide numerous health benefits. Prebiotics are substances from types of carbs (mostly fiber) that humans can’t digest. The beneficial bacteria in your gut eat this fiber.”

The Harvard Medical School explains that our large intestine has 100 trillion good bacteria essential to our health called microbiome. These good microbes help maintain healthy bowel function. Each person has their own unique microbiome from birth and we add to the good microbes by the food we consume. However, a diet high in sugar and fat can negatively affect gut health by encouraging the growth of bad bacteria, allowing them to grow faster and colonize more easily, without as many helpful bacteria to prevent it from happening. 

Research shows certain foods like yogurt and kefirs with live, active cultures are beneficial to adding to our microbiome. Unpasteurized, fermented foods like kombucha, miso, sauerkraut and sour pickles contain healthy microbes but most packaged fermented foods are pasteurized, which destroys the microbes. The best way to get around that is to learn to ferment foods at home.

Experts say while there are many probiotic supplements on the market, it is not easy to find exactly what you need. Most probiotic supplements contain just a few bacterial strains, compared to the many stains naturally found in the gut. “Although there are many claims made by probiotic products, there’s now reliable, evidence-based support that certain probiotics — like Lactobacillus, Bifidobacterium (bacteria), and Saccharomyces boulardii (yeast) — are generally safe and helpful in specific conditions.”

Brain Games Boost Mental Fitness

Over the last couple of decades, researchers have found brain-training games may help improve attention levels, logic skills, and memory as well as enhance cognitive function in healthy subjects. Shaheen Lakhan, MD. Ph.D. FAAN, a physician-scientist and clinical development specialist and founding neurology department chair and curricular dean at California University of Science and Medicine writes the following: “The brain is just like a muscle – it thrives on exercise! As a neurologist, I’m thrilled by the incredible potential of brain games to help people flex their mental muscles, activating underused brain circuits to sharpen cognition and skills like focus, speed, and memory.”

Experts say effective brain games include Sudoku and crossword puzzles. With crossword puzzles, cognitive skills used include recollecting past verbal knowledge and memories. Sudoku relies on short-term memory, which is often one the first sign of Alzheimer’s disease, and requires the player to look ahead to plan out subsequent moves.

Android and iOS offer apps for brain games that are either free, include in-app purchases or require a subscription to open more games. One game is Elevate that centers on reading, writing, speaking and math; and allows you to customize training on areas you wish to focus on. Another game, Luminosity, was involved in a study that showed people who played the games for “15 minutes a day at least seven days a week for three weeks experienced improved attention and motor speed.”
