It is Important to Identify Memory Loss Issues Early

Medical experts have found that many older adults are not being screened for cognitive decline during routine physical exams and say it would be a good idea to do so.  While many of us would be shocked to hear devastating news that we have early-stage Alzheimer’s disease, doctors say it is important to identify memory loss issues early as available treatments are more effective early-on.  Moreover, if it is determined you do have some memory loss, there are steps you can take to help slow down or mitigate the risk.   

Additionally, if you are wondering if a cognitive test is expensive, Medicare does pay for an annual “wellness visit,” which is supposed to include a quick cognitive assessment to help identify some early warning signs of dementia.  From there your doctor can recommend a more thorough examination.  However, according to an Alzheimer’s Association survey, over half of the respondents said their healthcare provider has never asked about their cognitive health, let alone administered a test.  Less than a third said they had discussed memory and cognitive issues with a doctor.  Joanne Pike, an Alzheimer’s Association chief program officer, says while, “Many older adults say they’ve noticed changes in their mental abilities, “the majority of the time seniors are waiting for the physician to bring it up.” 

So, what does a cognitive assessment include?  To start with, your doctor will ask you directly if you have noticed any changes in your memory recently.  If you have a family member with you, the doctor may ask them if they noticed any problems.  You may be asked to take a short test, but often the assessment includes the doctor asking questions regarding your activities, your interests and how well you are functioning at home.  In fact, you may not even realize you are being cognitively assessed.

Facts About Falls

A fall can result in serious injuries especially for the elderly, who make up most cases, and can take a long time to heal.  Many even lead to permanent mobility issues that limit the person’s freedom.  The CDC reports that each year 3 million older adults are treated in the emergency room for a fall injury and over 800,000 patients are hospitalized due to their injuries. 

Experts explain there are several reasons why the elderly are more vulnerable to falling and include medical conditions such as Parkinson’s and heart disease, arthritis, osteoporosis, hypoglycemia, diabetes, and blood pressure fluctuations.  In addition, physical disabilities that affect strength and balance, poor eyesight, inner ear problems, and dehydration can also put a person more at risk for a fall.  Common situations that can result in a fall include wet or recently polished floors, poorly lit areas, rugs not properly secured, stairs, and uneven sidewalks. 

There are some changes you can make to fall-proof your home such replacing light bulbs in dimly lit hallways or stairways with higher wattage bulbs.  Clean up clutter and make sure each room in your home has a clear, clean path to walk through and install grab bars and handrails where needed.  Use a non-slip mat in the bathtub or shower and do not wear socks or loose, floppy slippers but rather a good walking shoe with support and cushioning.   

If a loved one is unsteady on their feet, talk to their doctor about using a walker or cane.  Also, ask if any of their medications can cause dizziness or light-headedness, and if so, is there an alternative.  Doctors say if not, it is even more important to make the appropriate lifestyle changes.

A Healthy Diet Plays a Role in the Function of Our Immune System

Over the last several years, we have seen firsthand the benefit of having a robust immune system, which is vital to helping resist and combat diseases and illnesses, and a healthy diet plays a role in the proper functioning of our immune system. While food alone doesn’t directly boost the immune system, experts explain “a well-rounded diet can help prevent nutrient deficiencies that can hinder immune function, which in turn helps to ensure our immune system functions smoothly.” 

We all need certain vitamins and minerals for our immune cells to grow and function; and nutrient-dense foods help protect our bodies in the event of an invasion of bacteria, fungi or viruses. Likewise there are foods known to be harmful to the immune system.  

Alcoholic beverages are one of those things. If you do drink, it is recommended you limit your daily intake to no more than 12 oz. of beer, 5 oz. of wine and 1.5 oz. of liquor. Another thing to avoid is caffeinated foods or beverages too close to bedtime; and overall the FDA recommends no more than 400 milligrams of caffeine daily. 

Foods high in sugar and salt should also be avoided as well as fast food and processed foods loaded with empty calories, fat and sodium that offer little in the way of needed vitamins and minerals. Dairy products can be problematic for some individuals and lead to inflammation, which in turn affects the immune system. Experts say another good rule is to avoid snack treats with lots of food additives, colorings, and preservatives, which are associated with inflammation, and weaken the immune system.

The Importance of Eating Healthy Proteins

Information on what constitutes a healthy diet continues to evolve as researchers discover more and more about the human body and how it reacts to the food we consume and the way it affects the aging process. Not too long ago, medical professionals touted diets low in fat and high in carbohydrates. Now newer studies emphasize the role protein plays in our overall health and the importance of eating healthy proteins.   

Experts say the best proteins should contain vitamins and minerals. Cesar Sauza, clinical nutrition manager of health education and wellness at AltaMed Health Services in Los Angeles, explains his "top go-to protein foods sources are fatty fish like salmon, tuna and sardines.” A 3 oz. A serving of tuna steak contains 24 grams of protein and salmon 17 grams of protein. Fatty, cold water fish also contain omega-3s; fatty acids that help maintain eye and brain health as well as heart and circulatory health. Omega-3s DHA and EPA are linked to brain and cardiovascular health. 

Another item back on the menu is eggs. Cage-free eggs are a good source of protein (one egg has 6 grams), fairly inexpensive, easy to prepare and provide several essential vitamins and minerals including vitamin A, vitamins B2, B5, B6 and B12, folate, phosphorus and selenium.

Bean and legumes are other easy ways to get your daily dose of protein and are rich in fiber, vitamins and minerals, and antioxidants. They can help you lose weight as they contain a carbohydrate known as “resistant starch," that like fiber, is hard to digest and makes us feel fuller longer.

Other healthy suggestions for protein include low-fat dairy products. Along with the probiotic benefit, Greek yogurt has almost twice the protein as regular yogurt along with calcium and is typically fortified with vitamin D. Nuts and seeds are also loaded with protein, vitamins and minerals. Dieticians say if you eat meat, concentrate on lean meat and avoid processed ones like bacon, ham, deli meats, and hot dogs.

Be Careful When Using the Internet for Medical Advice

Let’s be honest, many of us rely on the Internet for medical advice. While it can supply good answers, doctors also caution you have to be careful what sites you trust. There are many websites providing good medical information but there are also sites with misleading or even dangerous advice.

Dr. Michael Langan, an internal medicine physician at Ohio State University Wexner Medical Center, cautions anyone with a blog can dispense information and advice; and a good way to avoid misinformation is to ask your doctor to recommend the sites they trust. Additionally, the National Institute on Aging recommends using health websites sponsored by Federal Government agencies. They say a good place for reliable, up-to-date information is the National Institutes of Health, but at the same time, remind everyone that the Internet is no substitute for seeing your doctor or other health professional who can give you advice that caters to your specific situation. 

Other things to look for include name recognition such as the Mayo Clinic or the Cleveland Clinic. Moreover, remember sites that end in .gov, identity U.S. government agencies; .edu, educational institutions; and .org, usually designates nonprofit organizations (such as professional groups; scientific, medical, or research societies; and advocacy groups). A .com is a commercial site.

The NIH says to look for the date of the information (often can be found at the bottom of the page) and see if the authors and contributors are identified. If the author is listed, what is his/her background and what is their goal in writing the article or the intent of the organization it is written for. Is there any financial motive for the information? Check out the About Us or Contact Us pages as they provide background information about the organization.

Preventing Falls in Older Adults

The Centers for Disease Control and Prevention (CDC) reports that each year, one out of every five Americans aged 65 years and older has a fall that results in an injury such as broken bones or a head injury.  However, medical professionals say while it is important for seniors to take precautions to prevent falls as injuries can be serious, they also remind us that regardless of how old we are, it is beneficial for our overall health to keep moving.  Experts explain that failing to engage in “mild exercise on a regular basis results in reduced muscle strength, decreased bone mass, loss of balance and coordination, and reduced flexibility.”

They go on to say that despite commonly held beliefs, falls are not a normal part of aging and there are things you can do to help keep you on your feet and mitigate the risk of a fall.  Their first advice is to talk to your doctor about your risk and prevention.  Tell them if you have fallen and discuss your medications to see if side effects include dizziness or sleepiness that could contribute to falls.  

While falls are common in seniors 65 years of age and older, individuals with dementia are at an even greater risk.  Researchers say there are several reasons for this including gait and balance issues as well as problems concentrating and with staying focused on the task at hand.  Other issues that may increase the risk of falls include poor eyesight and chronic diseases such as Parkinson’s, and arthritis or numbness in the feet.  If you often feel dizzy or lightheaded upon standing or suddenly sitting up, ask your doctor if you might have postural hypotension, and find out how you can alleviate the symptoms.    

Further tips to reduce your risk include having your eyes checked annually and updating your glasses when needed.  In addition, talk to your doctor about the best footwear for inside the house as well as outside.  Check your home for trip hazards (the CDC has a list of tips) and install grab bars in the bathroom.  Finally, ask your doctor about exercises to strengthen your leg muscles as well as balance-training exercises to improve balance.

Does Your Diet Increase the Risk for Dementia?

The Medical News Today says dementia is one of the leading causes of disability and death worldwide. The debilitating disease has no cure; and available treatments only alleviate symptoms without slowing the progression of the disease. According to the World Health Organization (WHO), dementia affects more than 55 million people globally, and 10 million people receive a diagnosis each year. Of that number it is estimated 60 – 70 percent have Alzheimer’s disease, and 6 million of them are Americans. Unfortunately those numbers are expected to rise to 139 million people across the globe by 2050.

Other rising numbers are in cases of diabetes, obesity, and heart disease, which are all believed to increase a person’s risk for dementia. This is why researchers are not only trying to find a cure for Alzheimer’s and other dementias but ways to reduce people’s risk for the disease. 

Dr. Christopher Weber, director of global science initiatives at the Alzheimer’s Association, noted that: “Research looking at the relationship between diet and cognition is well-established. There is strong evidence to suggest that what is good for the heart is good for the head, and we know a healthy diet is good for the heart.”  

Researchers say one diet that appears to suggest better cognitive health is the MIND diet, (Mediterranean-DASH Intervention for Neurodegenerative Delay). It is similar to the Mediterranean diet with a heavy focus on green, leafy vegetables, nuts, berries, bean, whole grains, seafood, poultry and wine. 

On the other hand, foods high in refined carbohydrates may cause an increase in the risk for Alzheimer’s. Chances are if your parents are elderly, they are taking at least one or more prescription medications daily.  In fact, the Washington Post cites research that estimates, “25 percent of people ages 65 to 69 take at least five prescription drugs to treat chronic conditions, a figure that jumps to nearly 46 percent for those between 70 and 79.”  Polypharmacy refers to the regular use of five or more prescription medications, and puts people more at risk for adverse drug interactions, an area of special concern for the elderly.  

Alarming figures from the Centers for Disease Control and Prevention state almost 450,000 older adults (65 years old or older) visit the emergency room each year because of adverse, or harmful, drug events.  Doctors say while many of these problems are preventable, when you take a number of prescriptions, it can get confusing keeping up with various instructions such as “take on an empty stomach,” and the adverse side effects. 

Experts say to help a parent get organized, write down the names of all of the drugs they are taking, the dosage, and if they see several doctors, who prescribed them.  Include any herbal supplements, multi-vitamins and over-the-counter drugs.  The next time they see their primary care doctor, take your list and discuss the reasons for each drug, how long they will be on it, and if any of the drugs are dangerous taken together.  Whenever a new drug is prescribed, ask what it is for and how it should be taken, and read through the paperwork.   

Store medications together in a cool, dry location (not the bathroom), discard old, expired medications, and find a pill organizer that makes sense for your parent’s needs.  If they are forgetful, they may need a reminder system.  However, if a parent is suffering from Alzheimer’s disease or another form of dementia, they should be carefully monitored.

In an article in the US News & World Report, experts say forgetfulness is not an inevitable part of growing older and there are things we can do to help maintain and enhance our memory function and overall brain health.  Furthermore, they also benefit whole body health.      

One of the first things they recommend is to never stop making “physical activity a priority as it optimizes blood flow to the entire body, including the brain.  An adequate supply of blood and oxygen to the brain helps to keep our memories sharp.”  Researchers explain that the type of exercise is also important and recommend a minimum of 30 minutes a day of cardio exercise such as running, power walking, biking, or swimming.  However, before beginning any new exercise regime, be sure to check with your healthcare provider.  

In addition, continuing to learn new things can improve our memory as studies have found a correlation between challenging our brain and maintaining brain health.  This could include activities such as playing an instrument, starting a new hobby, learning a language, or just broadening your reading habits.

Learn More About Care Before and After Surgery

Like most everything else, standard procedures and guidelines for care before and after surgery have evolved over the years to promote and speed recovery.  Dr. Charla Fischer, an orthopedic surgeon at NYU Langone Spine Center in New York City, suggests you think of surgery as you would a marathon or race.  She says, "The surgery itself is just the first part of the race – there is also the recovery after.”  So what you do before a planned surgical procedure and what you do after can all be part of a better outcome and even includes diet and exercise.   

One example is a change in the way a person is treated after surgery in many hospitals.  Now, rather than limiting patients to ice chips and intravenous fluids, they are served a meal.  In addition, people are encouraged to get up and move as soon as the night following surgery, which appears to hasten recover.  However, doctors warn that people should be screened before surgery for conditions such as malnutrition, anemia, diabetes and pain, and if it is found they have any, it should be addressed.  

Also, find out what exercises would be helpful and doable before and after surgery, and what is not recommended.  Ask about your hospital’s rehabilitation program or the availability of a physical therapist.   After surgery, stay hydrated and be sure to eat a protein-rich diet and lots of fresh vegetables.  Another thing that will aid your postoperative recovery and overall health is to stop smoking.   

As a bigger percentage of surgeries are in people over 65 years of age, your doctor will let you know the type of screening that is required before surgery.  It will most likely include things such as cardiac health, functional status and history of falls, frailty and other factors.
